Who we are looking for
We are seeking a skilled and empathetic individual to join our team as a Community Support Team (CST) Qualified Professional (QP). This role is crucial to the delivery of high-quality mental health and substance use services, helping clients achieve their recovery goals and improve their overall well-being. The ideal candidate will have strong clinical expertise, a passion for community-based work, and a commitment to making a positive impact in the lives of those we serve.
What you will do
As a CST Qualified Professional, you will be responsible for providing direct care and coordinating services for individuals with mental health and substance use challenges. Working under the supervision of the CST Team Lead, you will play a key role in assessing client needs, developing treatment plans, and ensuring that all services are delivered effectively and in accordance with best practices.
Your responsibilities will include:
- Conducting comprehensive assessments of clients to identify strengths, needs, and goals.
- Developing, implementing, and monitoring individualized treatment plans in collaboration with the client, family, and CST team members.
- Providing direct therapeutic interventions, including individual, family, and group therapy, as well as crisis intervention when necessary.
- Coordinating and linking clients to community resources, including housing, employment, education, and healthcare services.
- Advocating for clients within various systems, such as healthcare, judicial, and social services, to ensure they receive the support they need.
- Monitoring client progress and adjusting treatment plans as needed to ensure positive outcomes.
- Maintaining accurate and timely documentation of all services provided in accordance with agency policies and regulatory requirements.
- Collaborating with other CST team members, including the Team Lead, Peer Support Specialists, and other professionals, to ensure a holistic and integrated approach to care.
- Participating in regular team meetings and case conferences to discuss client progress and coordinate care.
- Promoting the values of recovery, person-centered care, and cultural competence in all interactions with clients and team members.

- A Bachelor’s degree
- A minimum of 2 years of experience working with individuals with mental health or substance use challenges, preferably in a community-based setting.
- Strong knowledge of evidence-based practices in mental health and substance use treatment.
- Excellent communication, organizational, and problem-solving skills.
- A commitment to person-centered care and cultural competence.
- A valid driver’s license and reliable transportation.
