Essential Functions
- Assumes responsibility for the effective coordination of patient/caregiver care.
- Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
- Make visits to provide appropriate interventions. Follows established plan of care, evaluates patient/caregiver responses to determine progress toward goals, and revises plan of care as necessary.
- Develops, implements, and evaluates plans of care for patients/caregivers. Incorporates therapeutic, preventive, and palliative nursing actions, and specifically addresses patient/caregiver education. Evaluates and updates plan of care as goals and objectives are achieved or changed.
- Emergency triage by telephone and home visits.
- Provides interventions consistent with the plan of care and performs all interventions required for each visit. Identifies new problems or needs when they occur.
- Facilitates patient and caregiver independence to extent possible.
- Initiates and maintains a liaison relationship with healthcare providers to effectively coordinate multidisciplinary care. Consults with the patient, caregivers, physician, facility staff and interdisciplinary team when initiating care and when significant changes in care needs occur. Reliably communicates pertinent information to appropriate members of the interdisciplinary team.
- Identify and refer all patient/caregiver psychosocial needs to VIA Health Partners Medical Social Worker as appropriate.
- Identify learning needs of patients and caregivers and provide necessary instruction.
- Demonstrate nursing care procedures. Utilize teaching materials, tools, and written information appropriately.
- Encourage and allows time for questions and consultation.
- When providing hospice care, appropriately utilizes VIA Health Partners policies, patient related protocols, standing orders and guidelines, including Enclara Pharmacia MUGs; Medline, and ProHospice, as applicable.
- Indirect patient/caregiver care responsibilities.
- Maintain productive and professional relations with external agencies and healthcare providers.
- Store all supplies safely and ensure that work area is clean, secure, and well-maintained.
- Maintain appropriate utilization of resources to ensure cost-effective delivery of healthcare.
- Apply principles of infection control and ensure universal precautions in accordance with accepted professional practice.
- Keep clinical records current by synchronizing laptop information throughout the shift.
- Complete and submit all clinical documentation per VIA Health Partners clinical documentation guidelines.
- Maintain current medication profile as directed by physician. Document medication teaching: purpose, doses, route, frequency, actions, indication for use, side effects and interactions.
- Record time/mileage accurately.
- Responsible for related duties as required and/or assigned.

- Maintain current professional licensure/certification, (RN licensure, CPR certification and CEUs).

- Responsible for all material reviewed at Nurses Meetings, After Hours Meetings, and All Staff Meetings, whether in attendance or not.
May perform other duties as required.
Qualifications
Minimum Qualifications
- A Diploma or Associates Degree in nursing is required; Bachelor’s Degree preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
- Current CPR required; CHPN preferred.
- Minimum 2 years related experience. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
- Prior experience in working in an environment which focuses on the pediatric population is required for Kids Path™ Nurse with BCLS (CPR) training for both adult and child/infant required.
- Computer proficiency is required.
